"The goal of this website is to provide practical advice, tools, and resources on how to plan, implement, and use patient and client demographic data in health-care settings."

Hello,

I am a social worker employed in a hospital setting. In my report on each patient I work with, I include information on the patient’s financial situation, housing, education, and resources. I usually place this info under the heading of SDOH, but am wondering if there is a more organized way to do this, one that possibly provides a revealing snapshot of a patient’s situation from a SDOH perspective. Do you know of a format or template that might be of use in this situation?
