Subject: Plain Language Information - Determinants of Health

Hello,
I am interested in finding examples of plain language material about the
determinants of health designed for approximately a
Grade 6-9 reading level in English appropriate in a Canadian (Ontario)
context.  Also, is there any "new" phrase for the "determinants of health"
phrase that is recommended when developing material for this same reading
level or that you have found general audiences respond well to when
discussing these concepts.
Any help would be greatly appreciated.
