   Dennis Raphael and Connie Clement will initiate group discussion by
   describing their efforts to raise the profiles of SDOH. Working groups will
   discuss successes, challenges and making connections with media,
   broad activist/social justice communities, public health and health care
   workers, elected representatives, and academics.
