Funny you should ask about this because I have been encouraging a colleague
in Montreal to write about his experiences as a coach to people with severe
mental illness in a long term care facility. I sent him your request and I
hope he responds to you.
As a trainer and consultant I am a member of a list serve of trainers and
coaches that you can find at http://groups.yahoo.com/group/Training-Ideas.
Many of the postings are from coaches,, usually they focus on life or
business coaching but a number of them are specializing in ADD and ADHD.
They have made references to books on coaching for this population. You can
join the list serve and search through the archives or post your request.

Hi There,
Im interested in finding out more about exisiting programs or models where a
GP may be able to refer to a 'coach/ facilitator' to assist clients in
making lifestyle changes they may need to be making in relation to a chronic
condition etc. This persons role may be around assisting the client in
identifying their own goals and targets, and then helping develop, support
and encourage an individual program that would link and interconnect with
existing community facilties and programs where possible.
Any ideas/ leads etc around this would be mostly appreciated!
