Good morning all, 
As part of a presentation and workshop on writing good survey questions,
I am looking to refresh my resources and collect new real life examples
of bad survey questions. I'm sure that many of you, like me, get quite a
collection of surveys to respond to in your work and other lives. So,
I'm looking for the worst. I will need the question and if possible the
answer structure provided. Source of the question is not necessary
(though it does sometimes provide a good laugh).
