Our book Health Promotion in Canada: critical perspectives (O'Neill,
Pederson, Dupéré and Rootman, eds, Toronto, CSPI, 2007) covers a lot of
the things you mention below and has been written with undegrads in
mind. Dennis Rapahel's book on the social determinants of health
(Toronto, CSPI, 2004 I think) covers also a lot of revelvant ground.
