The knowledge of the social determinants of health is quite old, but it may
have been called differently.
For example William Henry Welch said in 1915:
"It is a well-known fact that there are no social, no industrial, no
economic problems which are not related to problems of health."
Flexner S and Flexner JT 1941. William Henry Welch and the Heroic Age of
American Medicine New York: Viking Press 1941, page

When did anybody start to use the exact term? I tried some library
searches on the Internet using the words social, determinants,
health in the title:
On the COPAC database ( http://copac.ac.uk/copac/) I found a few
interesting references, the oldest dating from 1957 :
Social determinants of health and fertility in the Gezira / [by] Tameem,
Omer Abdalla ; Sudan Gezira Board. Social and Economic
Research Unit. Social Research Section . 1983
Selected readings in the cultural, social and behavioural determinants of
health / edited by John C. Caldwell and Gigi Santow .
1989
Theory, practice, and application of prevention in environmental health
services : social determinants of human health : task force
reports / sponsored by the John E. Fogarty International Center for
Advanced Study in the Health Sciences, National Institutes of
Health, and the American College of Preventive Medicine . 1976
Conference on social and physical environment variables as determinants of
mental health / Department of Health, Education and
Welfare . 1957
While the term certainly became very popular during the 90's, it definitely
has been used before.
