Survey of Potential Intermediaries/Resource Organizations: Best Practices Portal for Health Promotion & Chronic Disease Prevention

Are you an intermediary?

Health Nexus is working with the Public Health Agency (PHAC)'s Best Practices Portal (http://cbpp-pcpe.phac-aspc.gc.ca/ ) to identify organizations that work closely with front-line practitioners in public and community health -- specifically in the areas of health promotion and chronic disease prevention.  We are attempting to map some of these 'connector' organizations who can 'push out' information about evidence-informed practice.  These organizations, termed "public health intermediaries", are viewed as important agents in building a primary audience for the Portal as a hub for evidence-informed policy and practice.
