can assist in suggesting ways to measure health beliefs, please reply to

I am wondering if folks are aware of an instrument that measures health
beliefs (based on the health belief model).  It looks like there are
several different instruments based on the nature of the health care
(cancer, heart disease, etc).  I am looking for something that focuses
on either using primary care or that has been widely  use to measure
general health beliefs.
