[image001.jpg][image002.gif]<http://www.mentalhealthcommission.ca/English/Pages/default.aspx><http://www.mentalhealthcommission.ca/English/Pages/default.aspx><http://www.mentalhealthcommission.ca/English/Pages/default.aspx>[image003.jpg]

The Mental Health Commission of Canada (MHCC) is recruiting the following individuals to complete an anonymous online survey (5-10 minutes). If you choose to complete the survey, you will be eligible to win one of several $20 Gift Cards for Amazon.ca. We are looking for:

 *   Doctors
 *   Nurses
 *   Psychiatrists
 *   Clinical or Counselling Psychologists
 *   Social Workers
 * Allied Health Professionals (e.g., Physiotherapists, Occupational Therapists, Dieticians, Speech Language Pathologists, Audiologists, Pharmacists, etc.)
 * Graduate or Professional Students in ANY of the aforementioned disciplines

This survey is part of the Opening Minds Anti-Stigma / Anti-Discrimination Initiative of the MHCC. This short survey asks individuals questions about their attitudes to individuals with mental illness. It is completely anonymous and confidential. This is for the purposes of validating the survey tool and ensuring its reliability. This survey tool will be used to evaluate future programs aimed at health care providers.

If you are interested in participating, please access the survey via the following link:

Please help by forwarding this message to any individuals that may be eligible and willing to complete the survey.
