There is still a fundamental problem with the report. It assumes that 90%
of variance associated with heart disease is due to biomedical and
lifestyle factors.  This is only the case when you fail to include income
and/or education as relevant variables.  It has been known since the
mid-1970s (Whitehall studies and numerous ad nauseum studies since then)
that biomedical and lifestyle factors account for at best 20-30% of
variance in incidence and mortality from CVD.
