I like that resource too but it is pretty old now (late 90s IIRC).  

Our most recent SDOH resource is the revised (2008) edition of our Primer to Action, "A resource for health and community workers, activists and local residents to understand how the social determinants of health impact chronic disease--and what we can do about it".
