I believe that the Nurse Family Partnership program out of Denver, has a

Can any one direct me to resources on developing client/participant
contracts. This is for homevisiting prenatal program. We are very
conscious
of the fact that these contracts can be punitive or negative sounding.
Our
goal is to enable participants to accept responsibility regarding
their
health and set out some boundaries regarding service provided.
