Toba Bryant and I are writing a chapter entitled: "Models of Health, Models
of Care" and are looking for examples -- primarily Canadian but also UK and
USA -- of public health units that have been able to bring healthy public
policy and health care services into its day-to-day work on an integrated
basis.
Updated developments from Chinook, Interior Health, Waterloo, Liverpool
UK, San Francisco, Seattle (King), Minnesota, and Maine would be
appreciated!
