hi suzanne et al.

have you taken a look at what is being called the chronic care model of care? see www.improvingchroniccare.org

in psychiatry, we have moved now to a recovery model of care- which is focused on the idea that the direction of the work of learning to deal with/overcome an illness lies largely with the person with the illness/condition.  health care workers are their to help provide support and technical help (meds etc) in this process.

finally, in the uk i learned about a model of care called the health/opportunity ladder- which conceived of care as a support to help people pursue opportunities in their lives- so the goal was to provide care and ensure access to opportunities, broadly speaking..

please keep the list informed about what you learn about.

I too am interested, not only in health promotion in primary care, but
thinking further, an integrated approach to health care.  By that I mean
health promotion, mental health, social work and other real life needs
provided in the health care setting.  Does anyone have information on
writings or model programs in this area?

Greetings:

I work at the largest public hospital in the southeast (Grady Health
System), Atlanta Georgia.

Within our primary care center, we employ health educator.  They promote
wellness through several programs:

1. smoking prevention
2. exercise
3. nutrition
4. stress reduction
5. etc.

They are also employed in some of our community -based primary care centers.
To address our growing diverse populations, we do hire bilingual educators.

I have also hired them in the past as heath educators for breast/cervical
health programs.

Most of them have a MPH. We do get a lot of from Rollins School of Public
Health (Emory), and Morehouse School of Medicine.  

Please let me know, if you would like other information.

I am looking into the role of a health promoter, health educator, community
developer (or other related roles) within primary care.

Can anyone point me in the direction of primary care organizations that have
such roles?  I am already familiar with community health centre health
promotion roles within Canada are there any others our there?
