Working Together to Prevent and Manage Chronic Disease: Ideas, Innovation, and Insight - May 23, 2008
Chronic diseases account for the largest proportion of death and disease burden nationally and provincially. The prevention and management of chronic disease to improve population health is an Ontario health system priority.
The Ontario Hospital Health Promotion Network and St. Josephs Health Centre, Toronto, are pleased to host this one-day conference for health system providers from across sectors to work on practical relationships to provide an integrated, system response to chronic disease prevention and management. Featuring:

In a unique conference approach, participants will have the opportunity to work together on a detailed case study of one of four selected Chronic Disease Prevention and Management Successes:
* Diabetes in Vulnerable Populations: The London Ontario Experience ~ Betty Harvey
* Toronto Client Access to Integrated Services and Information (CAISI) Project ~ Dr. Tomislav Svoboda
* Using an Urban Health Framework for Health Equity Initiatives ~ Community Health Centres of Greater Toronto (CHC-GT)
* West End Oral Health Clinic ~ Partners for Parkdale Health Network
Participants will break into pre-selected workshop groups to examine the opportunities, barriers and practical steps required for local health service providers to work together, using the real world examples of these four successes.
