Diane,
  There are at the very least three places to start:
1. David Barker's "Fetal Origins" Hypothesis: inequalities in factors shaping fetal health produce unequal fetal outcomes.
2. The social determinants of health has a vast literature, including a Commission Report of the World Health Organization just released August 28, 2008, documenting the harsh health impact of poverty (inequity), particularly childhood poverty, on adult health including chronic illness.  See attached bibliographies that I've compiled. Also look for articles by Clyde Hertzmann and R. Poulton.
3. There is a research group focused on Adverse CHildhood Experiences. Their work does not deal with exposure to poverty. Rather, its focus is family violence and chaos in childhood having severe health impact in adulthood.  The two directors are Vincent Felitti and Rob ANda. I've attached an older bib' of their work.  By checking the names of the co-authors, you will find a number of post-2003 publications.

The Best Start Resource Centre is developing a new resource on child health, and would greatly benefit from your input and insights. 

I have been researching a report on the relationship between early childhod and the development/prevention of chronic disease.  The report will be designed for a range of service providers and policy makers who work in or provide support to the earl child development sector.  

Key components of the report will include:
- Literature review of existing research, highlighting the areas of strong correlation between early child development and the development/prevention of chronic disease. 
- The value and importance of addressing key determinants of health during the early years to prevent chronic disease
- Focus on specific examples of chronic disease (examples: obesity,cancer, diabetes, asthma and respiratory disease)
 - Key lessons from the literature
- Key recommendations for policy change
- List of relevant resources and references
