When confronted with that? I think you point out the faults of our
health and social services systems in the U.S. and press for policies
that make sense.

In the U.S., we could first start with having access to drug and alcohol
treatment for folks who can't currently afford it.

My second would be to have access to reproductive health services.

Comprehensive, medically accurate, age-appropriate sex education would
be a nice third. (Louisiana, which is the origin of the original post,
  is mostly if totally abstinence based.)

The Guttmacher Institute ranks Louisiana for contraception:

"Among the 50 states and the District of Columbia, Louisiana ranked

     * 34th in service availability;
     * 42nd in laws and policies;
     * 10th in public funding; and
     * 33rd overall."

http://www.guttmacher.org/pubs/state_data/states/louisiana.html

Fourth would be that vasectomy is much safer, more reversible, and
completely underutilized in the United States.

And, to point out, this isn't a new idea. North Carolina "led" the
nation for years with a sterilization program. Anyone so interested
will find this book on how women used NC's sterilization program to get
reproductive health services they were otherwise denied _and_ how many
were forced into sterilization they did not want:

Public Health and Welfare
