I'm not Dennis, but I think his point is that if you are poor or are otherwise on the wrong end of the determinants of health, even if you make "correct" choices -- don't smoke, exercise well, eat what you're supposed to eat, etc. -- you're still likely to have poorer health. Just making the "right" choices is not enough.

Actually, Its much more than this. Even if the choices are correct, no smoking, exercise, diet, health outcomes are poor for those who experience adverse social determinants of health.
