Dear Colleagues,

Is anyone aware of an existing health benefit plan structure that  
focuses on disease prevention and health promotion? What I mean by  
this, is that the benefits are "front loaded" such that the emphasis  
is on funding preventative services. This would be analogous to your  
typical dental plans that fund preventative dental services at 100%  
(e.g. hygiene, fillings), but fund only a portion of the cost of  
restorative services (e.g. crowns).

This is in the context of employer-paid benefit plans.
