Is anyone aware of an example of (or leads to) a "regional health plan" to address immigrant and refugee health?  

It could be one generated by a municipality, a health district or regional health authority, or a comparable body.  In terms of scope, it could be one that addresses some or several aspects of the continuum from prevention/promotion strategies to early intervention to treatment and rehabilitation.
