As a former marketing director, the answers to your question(s) are too
broad and generalized to be of any good.

Any sort of mass-mailing promotion needs to be handled by someone who knows
what they are doing; materials need to be designed by professional
typographers & designers to maximize readability, legibility, and
effectiveness, etc...; mass-mailings should be targeted to specific
recipients who have been identified as being the most likely to respond; and
lastly, mass-mailings need to be repetitive over an extended period of time
to be most effective.

As far as literature on this topic, there is plenty. Just visit your local
bookstore or library.

Again, exact details of a mass-mailing should be discussed in detail with a
promotional organization.

Mass-mailings have all been documented as being effective, if done right.
The key, is to do them right.
