Subject: Program Planning and Evaluation

I am part of a research team right now looking into program planning and
evaluation practices. I am writing to find out from you about the
practices in your workplace/health unit.

1.Do you have a corporate or departmental policy/procedure/mandate to do
program planning and evaluation? No - just as specified in the Ontario
Public Health Standards (OPHS)

2.Is program planning and evaluation being "done" constistently from
team to team, department to department? Yes and No. Operational Planning
is done annually. However, how they do it varies from program to
program. We do have a standard Operational Planning Tool.

3.Do you have standardized templates and forms that you use? Are they
mandatory? Operational Planning software use is mandatory, however, it
is often not used completely. We use logic model and the attached one is
an example that we promote from an evaluation perspective. We also
promote THCU products through our Logic model and evaluation workshops
for staff.

4. Do you follow the THCU planning model? If so, do you follow it
exactly or have you come up with your own version?

5.Are you using THCU templates? If so, which ones do you use? Have you
modified THCU templates? We often refer to and modify THCU templates

6. If you are using any other models and/or templates, what are you
using & where did they come from? We promote the logic model developed
by the University of Wisconsin, Extension in our workshops and often use
it in consultation with staff. However, it is not mandatory.

For Evaluation Planning I promote the use of a planning tool from the TB
Evaluation Working Group at the Centers for Disease Control and

7.In your department and/or team, who is responsible for "doing" program
planning & evaluation? Program managers have the responsiblity to ensure
it is completed. Often the work itself is delegated and shared among
program team members. Capacity and guidance is provided by an Evaluation
Specialist, Epidemiologists and Research Analyst

8. How are new staff made aware of the program planning & evaluation
practices in your workplace? Planning and Evaluation Team have an
orientation session, usually once a year. It is also referred to in the
agency-wide orientation for new staff

9. How are staff trained on the program planning & evaluation practices
in your workplace? Evaluation Specialist has provided Logic Model and
evaluation workshops

10. Is this training a one time event, annual, as needed, etc? Annually
or semi-annually - depending on need and requests. Also, program level
inservice training is available upon request.
