I'm trying to track down a report by Ron Labonte (1998) titled:

A community development approach to health promotion: a background paper on
practice tension, strategic models and accountability requirements for
health authority work on the broad determinants of health.  Unpublished
report prepared for the Health Education Board of Scotland.

If it was on the web an address would be great, as would an electronic
copy.

Any clues would be appreciated.

I'm also interested in any other documents that make practical
recommendations for local actions to address inequalities or health
determinants.
