Here are three interesting heart health opportunities. The due date for two
of the RFPs is JANUARY 14, 2002   Please pass this notice on to others you
think may be interested.

Toronto (South Region) Heart Health Partnership is seeking services for our
Multicultural Heart Health Program and for our Youth Leadership Program.
The following is a brief summary of three Requests for Proposals.  Please
assist us and pass this message on to parties whom you think may be
interested.

1. Request for Proposal:  Multicultural Heart Health Community Facilitators
Tool Kit
APPLICATION DUE: January 14, 2002

The Toronto (South Region) Heart Health Partnership is presently seeking
professional services to develop a facilitator's tool kit to be used by
community workers for planning and implementing heart health promotion
programs in seven multicultural communities. The tool kit will be in English
with sections culturally and linguistically adapted. The project does not
entail translation of materials.
The tool kit will incorporate the findings and recommendations of two
projects undertaken during the past two years: 1. An Inventory of
Multicultural Heart Health Educational Resources used in Ontario  2.
Identifying Best Practices for Promoting Heart Health in Multicultural
Communities
TIME LINES: January 28, 2002 - March 25, 2002

DELIVERABLES:  Draft versions of tool kit formats; final version of tool kit
including resources; Final written report

BUDGET/FUNDING: A limit of $7,500.00 (including expenses,  GST and PST)  has
been allocated for this work

2. Request for Proposal: Heart Health Youth Leadership Program Evaluation -
Phase 1- Peer Leaders
APPLICATION DATE: January 14, 2002
OBJECTIVE:To design and conduct a formative evaluation of the Heart Health
Youth Leadership Program, focussing on peer leaders,  in order to guide
future planning and implementation of the program.

BACKGROUND: The Youth Leadership Program (YLP) of the Toronto (South Region)
Heart Health Partnership is an after-school program for grades 6 to 8
students focussing on heart health promotion skills in peer leaders who, in
turn, develop and implement heart health projects in their school and
community.  This is a collaborative program that uses a partnership model
involving Toronto Public Health, Toronto Parks & Recreation, Toronto
Catholic District School Board and the Toronto District School Board. After
three years of program development and implementation, the YLP Workgroup has
completed a manual and developed a logic model.  The YLP Workgroup is
currently planning a formative evaluation of the program.  Phase 1 will be
an evaluation with student peer leaders.  Phase 2 will be an evaluation with
program partners, to begin in April 2002.  A separate RFP will be posted for
Phase 2 to accommodate budgeting. You may submit an application for one or
both Phases.
TIME LINES:  January - March 2002

DELIVERABLES: Evaluation tool for student peer leader focus groups,
completed application forms for Ethics Review - TPH, TDSB, TCDSB, completed
set of focus group data and summaries of each focus group session, final
report detailing methodology, analytical results and
interpretations/recommendations.

BUDGET/FUNDING:  $7000.00 has been allocated for this work (including
expenses, GST and PST)
