James, i would suggest you contact some staff at the McMaster University
who have published on this topic. This was done through the Public
Health REsearch & Education Program, a joint effort with the
Hamilton-Wentworth Health Department. Unfortunately, I don't have the
individuals' names with me, there are in my home office, but once I'm
there, I'll send them to you directly. LT
