The following message had trouble getting through to click4hp listserver, so
I'm posting it on Harvey Skinner's behalf.  To avoid problems with the
listserv when replying to messages, please cut out the header (the "To"
 "From:" and "Subject" parts)  of the message you are replying to, as the
list-serv reads them as commands and rejects them...  Alison

 If you could use a computer administered version, then look at the
Computerized Lifestyle Assessment that evolved from over a decade of
research.
The CLA covers 18 health/social areas, gives graphic feedback at various
points, generates a summary of lifestyle strengths and concerns/risks, and
also
assesses readiness for change.
