You might want to check the health canada web site for a start - they've got a lot of good info on initiatives that involve good health promotion principles (collaboration, participation, meaningful involvement from the population of interest, stakeholder participation etc.).  There is also some good info on population health on that website.  Health Canada also has links to other HP websites that might be of interest to you.
