I would suggest you talk to the people at West Vic Division of General Practice http://www.westvicdiv.asn.au/ they have an enabler model that assists people referred from a GP to access physical activity opportunities and they are successfully integrating it into routine care. I hope this is useful. I would be interested to talk to you further about what you are doing in this area as we are investigating different ways of providing this interface and building referral systems in relation to GPS and physical activity. Any ideas/ leads etc around this would be mostly appreciated!
