Hi Diane: Wearing my 'hat' as Chair of the Ontario Hospital Health Promotion Network, I believe that there is a TON of activity going on in this area across the Province, particularly if you look through the 'lens' of Hospital-Community Collaboration. Much of the work is 'under the radar', as rarely is there one point person at either a hospital or at public health units that 'coordinates' or knows about all the collaborative work that individual health unit and hospital staff are doing in the community in the area of disease prevention, determinants of health, etc. For example, in Peel and Mississauga, we have a number of projects 'on the go' (either in development or already in the implementation phase) that are in partnership or collaboration with Peel Health, community agencies (e.g. Rec and Parks), and ourselves the hospital (Trillium Health Centre), that are related to chronic conditions and the integration of health promotion or chronic disease prevention. Here are just a few that I know of: E.g. Consumer Health Information Physician Channel Project (in partnership with Peel Health to distribute CHI in Physicians offices) E.g. Mississauga Child & Youth Obesity Partnership Project (in partnership with Miss. Rec & Parks, YMCA-Miss., Get Active Mississauga, Healthy Living Peel, Trillium Health Centre) E.g. Mississauga Consumer Health Information Consortium (in partnership with Peel Health, Mississauga Library, UofT-Mississauga, AstraZeneca, Trillium Health Centre, Credit Valley Hospital) E.g. Marginalized Women and Stroke Project (Peel Health, the Ontario Women's Health Network, community agencies serving South Asian women) E.g. Lay Health Educator Breast Screening Program Project (Peel Health, Trillium Health Centre, community agencies serving South Asian women) E.g. Smoking Cessation pilot partnership project in Cardiac Health System (with Peel Health) E.g. There are lots of comorbidities with mental illness such as cardiovascular and depression or bipolar and healthy weights.
