Community Health Workers: Where We Live Work and Play
DEADLINE FOR NOMINATIONS: April 1, 2007
The 2007 MWPHA Annual Public Health Awards will recognize individual and organizational achievements in supporting and promoting access to health care in the Washington, D.C., metropolitan area, including the communities of Virginia and Maryland. The awards will showcase the efforts and accomplishments of community members and diverse organizations, including agencies and programs; working outside of traditional official public health system, with a relationship to the local public health system, to promote health, and prevent disease and disability in our community. Contributions designed to support national and local objectives to promote access to health care are encouraged. The awards will be presented at the MWPHA Annual Conference on April 16, 2006, at the annual conference on April 16th (Community Health Workers: Where we Live, Work & Play). 2007 ANNUAL PUBLIC HEALTH AWARDS
ENTRY INSTRUCTIONS
Complete the Entry Form (Download PDF) (available on-line at www.mwpha.org)
Attach a description of the program, activity, or individual achievement (no longer than 3 typewritten pages) which summarizes the points below. Additional information or supporting documentation may be appended to your entry form, but judging will be based primarily on the content of the summary.
Background - Describe the need for the program, activity, or achievement or any special circumstances related to the development of the program or activity.
Objectives - List the major objectives of the program or activity.
Description - Outline the program or activity and major events, components, or accomplishments, outcome on workforce development, information system capacity, organizational capacity including public health laboratory quality and accomplishments. (For individual awards, please describe personal achievements, community involvement, and professional affiliations).
Results - Describe the effect of the program and results to date, providing quantitative data, where possible (for example, numbers of persons served or affected, evaluation measures appropriate to the program or contribution).
