Good Morning Lauri, I would also be very interested in reviewing this survey instrument and was also wondering if the original sample included smaller urban centers (< 30,000)? it be possible to have it posted somewhere? The survey has only been used on a small sample in Louisville and will be used in Milwaukee. Any interest in that? it is obviously not validated but we are trying to get that done. This will serve as a baseline and possibly for future follow up. We plan a random sample selected from one neighborhood in Miami. We plan a subsequent intervention which will longitudinally partner students from our new college of medicine with households throughout the students training and engage the students in service-learning in this community.
