[NPS Civil War Soldiers & Sailors System] [Kentucky death records] William Fuller Church, b. 28 Feb. 1818, Middlefield, Ma. 6 ft.tall, hazel eyes, gray hair, light complexion, "rather thin" face. [Passport Application for Travel abroad, 10 May 1867, made in Washington, D.C.] [Middle name from MA town birth records]; died of "Bronkitis", 15 Oct 1892, Huntington, MA; buried in Amhurst.
