You are invited to participate in a research survey exploring your general knowledge about music therapy.  The survey consists of six open-ended questions and should take between 10-15 minutes to complete. 

The survey is part of a research study exploring the current state of professionalization of music therapy in Ontario.  The study is being conducted as partial fulfilment of the requirements for the degree of Doctor of Philosophy in Health Policy and Equity at York University in Toronto, Ontario and has been approved by a York University Research Ethics Board. 

For the study, the researcher is interested in the perspectives of students and faculty members affiliated with the School of Health Policy and Management at York University.  As students, you are future decision-makers within the healthcare system and as faculty, you may already be involved in research and policy initiatives related to health and healthcare.  The researcher would like to learn about your opinions of music therapy and its role within the healthcare system.  

Participation in the research study is voluntary and there will be no remuneration for choosing to participate.  You can withdraw consent to participate at any time.
