Hi there,

Every year, Health Promotion Ontario recognizes and celebrates excellence in health promotion with our annual award – the Lori Chow Memorial Award.  We continue to recognize leaders in health promotion through this award.  *NEW THIS YEAR*, we are also recognizing health promotion projects or initiatives.

Individual Award

Do you have a friend or colleague that has gone above and beyond in their contributions to the work of health promotion?  The Lori Chow Memorial Award is presented annual to someone who champions the importance of the social determinants of health, demonstrates leadership in health promotion, and shares their passion by mentoring others.  The award recipient will demonstrate excellence in the field of health promotion.

Health Promotion Project/Initiative

New this year, HPO would like to recognize projects or initiatives.  We know that there is lots of great health promotion work happening in communities across Ontario that are making a difference!  This new category for the Lori Chow Memorial Award will recognize this excellent work.  If you know of, or have worked on, a health promotion project or initiative, you can fill out the nomination form on our website.  In your submission, please describe:

  • The project or initiative (the issue(s) being addressed, the goals or objectives, and the priority population);
  • The team that worked on the project;
  • How the community and/or stakeholders were involved;
  • Why this project or initiative is innovative; and
  • The impact or effectiveness of this project.

We are now accepting nominations.  Please submit by completing the nomination form on our website.  Submissions will be accepted until November 25, 2022.
