Good morning and Happy Monday!

Hope you had a lovely long weekend.

We have some exciting news to share with you.

First, we would like to offer a sincere thank you for participating in Ontario Health’s social determinants of health interviews and merit review survey – your feedback was incredibly valuable.

Second, we are excited to share that our Social Determinants of Health (SDoH) Framework is now available on Ontario Health’s website<https://www.ontariohealth.ca/system-planning/social-determinants-of-health-framework>. The SDoH framework was developed through extensive research and consultations – including with you – and includes eight key evidence-based principles. It is our hope that this framework supports a paradigm shift where social needs are incorporated into care planning at both the individual and population level.

Third, we are finalizing accompanying resource guides for each of the eight principles that will provide tangible examples and resources to action each one – we will share those once they’re finalized.

We hope you take some time to read through the framework and join us in championing holistic care for everyone in Ontario. Together, we can ensure better coordination and integration for improved health outcomes.
