You seem to have missed the single most important risk factor for heart
disease in the USA, Canada and other Western nations: low income!

Numerous studies indicate that the risk factors you identified account for only
a small proportion of the incidence of cardiovascular disease among
populations.

You may wish to look for the monograph: "Inequality is bad for our hearts:
Why low income and social exclusion are major causes of heart disease in
Canada"  [working title] which will appear in September, 2001. I will inform
