Barbara's point is terribly important. Here in the UK,
as list members may well know, we can look at health
according to a measure of social class based on
occupation for all ethnic/racial groups. We can compare
mortality between these groups back to 1921. This is
why the UK has had many reports and research
initiatives on "Health Inequality" . We did go through a
phase in the 1980s where we had to call it things like
"social patterning" or "health variation". We now have
a new measure of social class, to be used in all
official statistics, explicitly based on 'employment relations
and conditions', among which are job security, autonomy
over one's own work, and career prospects. Income is
not measured in UK censuses. What this means is
that we can at least start trying to attain a consistent,
coherent model of health inequalities that makes sense
of gender, sexual orientation, class, race, ethnicity. In
so doing, it does become clear how partial are some of
our explanations when we only look at one kind of
inequality and treat the others as invisible or nuisances
to be statistically adjusted-away. By introducing a
measure of social position that highlights relations
and conditons at the workplace, the new census
measure encourages researchers to look at the
experience of degrees of oppression of various kinds.
I think this may bring us nearer to a more consistent
explanation for all forms of health inequality, though
we have an awful long way to go.
