Living conditions are the primary determinants of whether people stay
healthy or become ill. Dr.
Dennis Raphael’s presentation will outline what is known about the social
determinants of health and reasons for the gap between rhetoric and action.

The Community Integration Seminar Series showcases new developments in the
fields of health promotion, population health with a focus on service
integration through seminars and panel discussions featuring practitioners,
policy makers and researchers. Seminar topics are practice-oriented, and
are relevant to both professional needs and the scope of the communities in
southwest Toronto. Community health planners, service providers,
practitioners, students, and all other interested parties are welcome to
attend these free seminars.
