Hello all,
I have seen some of the exchanges on potential names but haven't had time to
chime in. That said, my reaction is similar to Larry's...ie, straying away
from language that can only be de-coded by academicians. My suggestion would
be to opt for something that captures the attention of a broader spectrum of
individuals and groups. Hopefully, at the same time, those of us who tend to
fly at 30,000 feet (mea culpa) will still be able to appreciate something in
'plain language'! :-)
Another small suggestion, to add to the pile. I would consider a title or
subtitle that addresses the individual reader, regardless of social
status/position. Something like, "Social Causes: What You Might Not Know
About Your Health". That's just off the top of my head.
The virtue of this approach, to my thinking, is that it not only suggests
individual 'investment' and that this is about "me" the reader, but it also
acts as an entry point into the gradient dynamic of health
inequalities/inequities...so that even someone who is higher up the
socio-economic ladder (for example) is invited into a discussion about how
inequities effect them too. I believe strongly that while it is important
for us to militate in support of a social equity and health equity movement,
there's also something to be said for tapping into many people's willingness
to act, if only out of enlightened self-interest.

Sorry for the late entry into this discussion. Unless you're targeting an
academic audience, I would discourage use of the term 'social determinants'
in the title. Yes, I know, there really are not any adequate popular
alternatives. It took us months to come up with the name "Unnatural Causes:
Is Inequality Making Us Sick?" for our documentary series (which, btw,
you're welcome to use; Alameda County (CA) Public Health Dept, for example,
issued a splendid report called "Life and Death from Unnatural Causes"

Many suggest that titles for publications / programs / campaigns for change
should communicate a sense of possibility rather than alarm, impel not
compel, be about "us" not "them." Hope this helps.
