Depression is a Chronic Illness, and primary care doctors still have a
long way to go to properly manage it.  Even for those of us with high
interest in the social aspect of it, treatment is fragmented by lack of
psychological evaluation, limited resources, and lack of access (US-poor
populations).
Not to think of depression as an illness is a disservice to the
patient.

The IHI website is the best effort at using chronic care models for
diabetes mainly.
worth looking.
