I'd suggest that you try contacting the Voluntary Health Association of
India.  I don't have their address with me, but you could probably get it
through the Indian High Commission.  VHAI has state level branches in most
states -- and are highly involved in delivery of health services at primary
level.
 
Another possibility from within Canada would be to contact the South Asian
Partnership.  I don't have the address with me, but their office is in
Ottawa and should be listed in the phone book.
