Hi There,
Im interested in finding out more about exisiting programs or models where a
GP may be able to refer to a 'coach/ facilitator' to assist clients in
making lifestyle changes they may need to be making in relation to a chronic
condition etc. This persons role may be around assisting the client in
identifying their own goals and targets, and then helping develop, support
and encourage an individual program that would link and interconnect with
existing community facilties and programs where possible.

Any ideas/ leads etc around this would be mostly appreciated!
