Here in Hamilton-Wentworth (Ontario, Canada) we are working on a
"twinning project" with Niagara Region on a Heart Health in the Workplace
Project. We want to access the workplace in health promotion, but were
astounded to find out that the Heart Health projects around use
assessment instruments that have not necessarily been validated or
evaluated. Does anyone know of a questionnaire to administer in
workplaces for individuals (other than Health Canada's Workplace Health
System as this is too broad) but is not necessarily a health risk
assessment (HRA) as we see in the US? We are looking for something that
has been evaluated and validated for a health needs assessment.

We are also looking for an individual environmental assessment - to ask
questions like do you have a smoking policy, healthy eating policy , flex
time etc. other than the one put together in Nova Scotia??

Any help would be truly appreciated.
