Hello click4hp listmembers

Would anyone have a list of a few good references on file
to document the movement of patient care in Canada from the hospital
setting to the community?

I am writing a proposal for a university program to prepare
people to take on community responsibilities.  I thought
that the need was too obvious to require documentation but
apparently I was wrong.
